What Our Transitional Care Program Is All About

When a loved one is discharged from the hospital, the next step isn’t always simple or clear. Many families face a tough choice: bring them home before they’re truly ready, or scramble to find a safe place where they can recover with confidence and without pressure.

Our Staycation program offers a reassuring middle ground… It’s ideal for seniors who are recovering from surgery, illness, or injury and need time to rebuild strength before returning home. Maybe they require help with medication management, meals, and daily activities, or they’re honestly evaluating whether assisted living might be the best long term solution for everyone involved.

We understand the emotional weight behind these decisions. Nobody wants to feel like they’re rushing their loved one or making permanent choices during a vulnerable time. That’s why our program is designed to bring peace of mind, providing professional clinical support, a warm environment, and open communication every step of the way. After several weeks, residents either feel ready to go home, or they decide to stay with us long term. There’s no pressure either way.

Frequently Asked Questions About Transitional Care at Stratford Place

Transitional care, which we call our Staycation program, is short-term senior care that bridges the gap between a hospital stay and returning home. It is designed for seniors who are recovering from surgery, illness, or injury and need time to rebuild strength in a calm, home-like setting before deciding what comes next, without the pressure of a permanent decision.

Transitional care stays at Stratford Place are a minimum of 30 days, which gives residents time to rebuild strength and confidence after a hospital stay, surgery, or illness. After that, families typically feel ready to return home, or they decide to stay with us long term. There is no pressure either way.

One straightforward rate covers personal care such as help with bathing, dressing, and mobility; medication management; nutritious home-style meals; housekeeping and laundry; utilities; 24/7 trained caregivers; activities; maintenance; and a private studio in our single-level, easy-access building. There are no level-of-care charges or surprise fees.

Please contact our community at (843) 300-1951 for current pricing and availability. Whatever the length of stay, Stratford Place uses one all-inclusive rate with no hidden fees, rather than charging separately for medication management or higher levels of care.

Stratford Place is an assisted living and memory care community, not a Medicare-certified skilled nursing facility, so Medicare does not cover transitional stays with us. At this time, we accept private pay and long-term care insurance. Call (843) 300-1951 and we will help you understand your options.

It is ideal for seniors who have been discharged from the hospital but are not quite ready to live independently at home, those recovering from surgery or illness who need help with medication, meals, and daily activities, and families who want a safe place for a loved one while they evaluate whether assisted living is the right long-term solution.

It starts with a phone call or a referral from a hospital discharge planner. We complete a quick assessment of your loved one’s recovery needs, review what is included in the all-inclusive rate, and coordinate a smooth move-in, often on short notice, so there is a safe place to land right after discharge.

Yes. If a resident decides that assisted living or memory care is a better fit than returning home, the transition is seamless. Both buildings share the same single-level layout, so familiarity and comfort stay intact, and there is never any pressure to decide before they are ready.

Bring hospital discharge instructions, a current medication list, comfortable clothing for recovery, and any mobility aids your loved one uses. Bring your questions, too. Our team will guide the rest and help your loved one settle in quickly.

Stratford Place is a small, family-owned community of 28 private studios, so every resident, including those here for a short transitional stay, is known by name and cared for by a consistent team. That small scale is what makes recovery feel personal rather than institutional.
